PCQN provides a variety of forums for members to learn, share experiences, and support one another in efforts to improve palliative care services provided to patients and their families. Descriptions of the various ways people can engage with the PCQN are below.
Quality Improvement Collaborative
The PCQN QI Collaborative supports teams to learn QI methods and work together on coordinated, mentored QI initiatives. Participants in the QI Collaborative join monthly conference calls during which the QI framework used for a particular project is reviewed (e.g., the Plan-Do-Study-Act cycle11); teams examine their data, and share their progress, successes, and challenges encountered since the previous call. Ongoing mentorship and support is provided to teams between calls, as needed. The QI Collaborative focuses intensively on one project for six months to one year.
Monthly Educational Webinars
Monthly PCQN educational webinars focus on PC clinical content that is directed at PC specialty clinicians. Presentations are largely given by PCQN members with non-member experts brought in periodically. Topics have included treatment of challenging pain conditions, pharmacist interventions & outcomes, and palliative care for vulnerable populations, among many others. CE credits are available for all participants.
Longitudinal Learning & Strategy Exchanges
Longitudinal learning and strategy exchange opportunities allow members to have more in-depth discussion on a particular topic with calls and/or webinars that are held on a regular, ongoing (e.g., quarterly, monthly, etc.) basis over a set period of time (e.g., 1 year). These are different from the monthly QI collaborative calls as they don’t necessarily have to do with using PCQN data to drive improvement activities. Depending on the nature of the topic, these sessions are open to all who would like to attend or are limited in size and available on a first come, first serve basis.
Upcoming Longitudinal Learning & Strategy Exchanges are:
- Collecting Quality Data in Community Based Settings – This is intended for all members who are collecting data in outpatient clinic or community based settings to raise questions & share strategies for data collection, and discuss desired analytics.
- Resiliency Training – To begin Winter 2018. More information coming soon.
Case reviews provide members with another mechanism for learning from one another. Members will have the opportunity to present challenging or unusual patient cases and engage in discussion with their peers. These are held in webinar format and be one hour in length.
The two, two-day in-person conferences per year aim to foster a sense of community while creating opportunities for members to share expertise and learn. Conferences include sessions on clinical topics, QI education and best practices identified through the work of the QI Collaborative, and sessions on leadership and self-care to further support the personal and professional development of participants. The conferences are highly interactive with built-in structures that facilitate the exchange of ideas and expertise on key issues faced by PC teams. Up to four individuals from each member organization can attend at no charge. Continuing education units are available for participants.
